After a year piloting the Sunday Primary Care Clinic for Farmworkers at Coastside Clinic, HCH/FH is entering year two by adding Puente and Coastside Hope as referral partners. The goal is to reduce no-shows, better understand demand, and possibly adjust capacity while currently limiting appointments to Medi-Cal-eligible farmworkers and families. This ongoing service model may drive needs for outreach support, scheduling and reminder tools, transportation coordination, and later expansion to other coverage groups.
Clinic currently limited to Medi-Cal eligible farmworkers and their families, with exploration of ACE and uninsured coverage options.
