Hospital Development of West Phoenix Inc, doing business as Abrazo West Campus and Abrazo Buckeye Emergency Center, plans to enter into a two-year ambulance services agreement with Arizona Fire & Medical Authority for emergency and non-emergency patient transportation. Hospital-responsible transports will be billed at Arizona Department of Health Services rates with a 30% discount when invoices are paid within 45 days, followed by a possible six-month month-to-month extension.

Renewal

Initial two-year term commencing on the Effective Date, with automatic month-to-month renewal for up to six (6) months if neither party executes a renewal, extension, or provides notice of intent not to renew or extend. Either party may terminate without cause on 30 days’ written notice or for breach, changes in law, or conflicts of interest under A.R.S. § 38-511.
