SUMMARY
The Town of Enfield is seeking proposals from qualified consultants to provide process and planning facilitation services for the Town Department of Social Services and the resultant Mental Health & Wellness Community Coalition. The consultant will facilitate the design of the coalition and its community plan to address conditions related to improved mental health and wellness.
EVALUATION CRITERIA
Background and experience in providing similar services, specific background/education/qualifications/relevant experience of key personnel, references, technical understanding of scope of services, proposed fees, and other relevant criteria.
REQUIRED DELIVERABLES
Support Director of Social Services in convening a collaborative, Conduct analysis of strengths/weaknesses/opportunities/challenges, Develop team charge and governance structure, Inventory existing committee/coalition efforts, Collect and present data, Write a 3-year action plan, Draft a Sustainability Plan, Create agendas for and facilitate/document planning meetings and convenings.
REQUIRED ATTACHMENTS
Letter of Transmittal, Background and Experience, Project Understanding and Implementation, References, Fee Proposal, Written affirmation that the Proposer has read and accepts the RFP's conditions, the Town's standard contract and conditions and the Town's contract compliance requirements in their entirety and without amendment.
