Biometric Screening $65 per person per event (25 person minimum); Provider Screening Form $15 per form per month; Lab Voucher (LGH Lab, Off-site, or Quest) $65 per form per month; Wellness Portal + Online Health Assessment $7 per person per month; Flu Shots varies per person per event; Optional add-ons: Empty Seat Fee $65 per missing participant; Presentations $200 per hour ($150 for screening clients); Learning Stations $80 per hour; Chair Massage $100 per hour/provider

Initial term is August 1, 2026 through July 31, 2027. Either party may terminate for breach with 30 days’ notice and opportunity to cure, or without cause with 30 days’ prior written notice; if the employer is paying incrementally, remaining balances for the initial term must be paid upon early termination. Future renewals or extensions would require a written amendment signed by both parties.
