One-Year Option: flat fee of $7,000; Three-Year Option: annual flat fee of $7,000 each year

Renewal and/or amendments may be made upon mutual agreement of both parties prior to the contract’s expiration. Fees may be adjusted if the district participates in Medicaid expansion and receives increased reimbursements; any fee changes require mutual agreement.
