$12,500 flat fee (one-year option) or $12,500 per year for three years

Agreement may be renewed or amended upon mutual agreement of both parties prior to expiration. Fees may be adjusted by mutual agreement if the district participates in Medicaid expansion and receives increased reimbursements. Contract may be terminated for material breach if not cured within 60 days after written notice.
