Perkins Schools Plans Medicaid Program Services Contract

$5,500 flat fee (one-year option) or $5,500 per year (three-year option)

PROFESSIONAL_SERVICES

Either: July 1, 2026 – June 30, 2027 (one-year service agreement) OR July 1, 2026 – June 30, 2029 (three-year service agreement)

Agreement may be renewed or amended upon mutual agreement of both parties prior to expiration. Fees may be adjusted by mutual agreement if the district participates in Medicaid expansion and receives increased reimbursements. Either party may terminate early for material breach after written notice and a 60-day cure opportunity.
