Summary

Rio Arriba County Health and Human Services is seeking a qualified individual to fill a position as Case Manager for the SAMHSA Naloxone Project. The Case Manager will be responsible for providing OUD Prevention Outreach, community partnerships, presence at community events, and cultural practices.

Evaluation Criteria

1. Provide appropriate certification(s) for your area of practice (25 points), 2. Discuss your outreach experiences? How do you plan to employ these experiences to advance project goals? (25 points), 3. Describe your experience working with underprivileged population? What are some of the strategies you've employed in your past outreach activities to advance inclusivity? (25 points), 4. Provide your hourly rate for services. (25 points)

Summaries of prevention outreach activities, community partnership activities, monthly reports on activities, trainings, and distributions.

Section 4.2 — Scope of Services and Deliverables

The selected vendor shall provide all labor, materials, equipment, and supervision necessary to perform the services described herein. Work shall commence within fifteen (15) calendar days of the Notice to Proceed and shall be completed within the timeframe specified in the project schedule attached as Exhibit B.

Proposals must include a detailed implementation timeline, staffing plan with key personnel qualifications, and three (3) references from comparable government engagements completed within the past five years. Evaluation will be based on technical approach (40%), experience and qualifications (30%), cost proposal (20%), and small business participation plan (10%).
