REVISED REQUEST FOR PROPOSAL (RFP) BELLERIVE ACRES SEWER REPAIR/IMPROVEMENT

This revised Request for Proposal (RFP) is for the Bellerive Acres Sewer Repair/Improvement project. The City of Bellerive Acres is seeking proposals from qualified vendors for sewer street repair services. The submission deadline is February 28, 2025.

Quality, availability, and contractual services to the particular use intended; The ability, capacity, and skill of the bidder to perform the contract or provide the service required; Whether the bidder can perform the contract or provide the service promptly, or within the time specified, without delay or interference; The character, integrity, reputation, judgment, experience and efficiency of the bidder; The quality of performance of previous contracts or services; The previous and existing compliance by the bidder with laws and ordinances relating to the contract or service; The sufficiency of the financial resources and ability of the bidder to perform the contract or provide the service; The ability of the bidder to provide future maintenance and service for the use of the subject of the contract; The number and scope of conditions attached to the bid; Whether the bidder is in default on the payment of taxes, licenses or other moneys due to the City.

Affidavit of Work Authorization, Affidavit of OSHA Training compliance, Affidavit of Compliance with the Prevailing Wage Law, Affidavit of American Products Purchase, Final Pay Affidavit, Non Collusion Affidavit, E-Verify, Certificate of Insurance and Endorsement, Supplier Diversity Utilization and Subcontracting Plan, List of certified MBE/WBE/VBE/SDVBEs, List of references for similar design projects, List of any and all portions of the project to be sublet, company's certificate of liability insurance, Professional Errors and Omissions Insurance, audited financial statement, or an annual report by a certified public accountant, evidence of bankruptcy/reorganization, evidence of sanctions/investigations, list of equipment or services required of a subcontractor, detailed list of expected costs or expenses, summary and explanation of other contributing expenses, brief summary of total costs, construction bond.
