Summary

This Call for Proposals seeks projects that address advancing health equity by addressing health disparities and adverse Social Determinants of Health (SDOH), with a particular emphasis on groups disproportionately impacted by the COVID-19 pandemic, and promoting the efficient and strategic delivery of integrated health and social services through innovative, sustainable community partnerships. Projects must focus on one of three areas: chronic disease prevention and management, family health (including maternal and child health, and childhood asthma), or behavioral health (including mental health and substance use disorder and the opioid crisis).

Evaluation Criteria

Strategic priorities, innovative/sustainable/replicable projects, community need, project impact and prospects for success, project monitoring/evaluation/capacity to collect/report data, participation of stakeholders and partners, organizational commitment and financial viability, workforce diversity, cultural/linguistic/health literacy competency.

Regular project progress and fiscal/expenditure reports, Deliverables produced under the grant, Final written report.

Letter of Intent, Financial Audit, Memorandum of Understanding (if applicable), IRS determination letter, List of officers and Board of Directors, Organizational Chart, Overall organization budget, Form 990 (if applicable), Résumés of key personnel, Logic model, Letters of commitment from collaborators, Service maps (optional), Annual report (optional).

Section 4.2 — Scope of Services and Deliverables

The selected vendor shall provide all labor, materials, equipment, and supervision necessary to perform the services described herein. Work shall commence within fifteen (15) calendar days of the Notice to Proceed and shall be completed within the timeframe specified in the project schedule attached as Exhibit B.

Proposals must include a detailed implementation timeline, staffing plan with key personnel qualifications, and three (3) references from comparable government engagements completed within the past five years. Evaluation will be based on technical approach (40%), experience and qualifications (30%), cost proposal (20%), and small business participation plan (10%).
