Service fee equal to 36% of increased SBAP/MAC reimbursements in FY 2026-2027, 33% in FY 2027-2028, 30% in FY 2028-2029, plus optional Medicaid Practitioner Authorization Form fees of $15 per student for therapy-related authorizations and $28 per student for other services; $5,000 per month from Effective Date through June 30, 2029, reconciled annually
