Effective as of the date of the Service Agreement and continuing until terminated by the Covered Entity or expiration/termination of the Service Agreement

Agreement continues for the life of the underlying Service Agreement and may be terminated by the Covered Entity for material breach or upon expiration/termination of the Service Agreement; certain privacy and security obligations survive termination and PHI must be returned or destroyed where feasible.
