Not to exceed $8,000.00; $1,400 for the first Form 471 Funding Application and $1,100 for each additional Form 471, plus an additional 2% of USAC approved reimbursement for Category 2 and Category 1 Dark/Self-Provisioned Fiber requests, capped at 100% of E-rate reimbursement per Funding Request

Agreement is limited to the 2026 E-rate Funding Year; any work beyond the specified scope or future funding years would require a supplemental or new agreement. Invoicing and reimbursement processes under this agreement may continue through as late as January 2028 based on USAC rules.
