NYC Health + Hospitals launched a kidney transplant navigator pilot in 2025 to address gaps in transplant access after removing race from eGFR calculations and joining the Coalition to End Racism in Clinical Algorithms. Because H+H is not itself a transplant center, the navigator in Brooklyn focuses on outreach to transplant candidates, assisting with workup completion and insurance issues, and liaising monthly with local transplant centers to identify and overcome barriers. In its first year the navigator recorded 154 patient engagements and 677 encounters, and data show that 16 percent of these patients lack a regular PCP or nephrologist, highlighting an opportunity for system re‑engagement of a medically complex cohort.The pilot is currently geographically and clinically limited, and its evaluation—supported by a Doris Duke grant—may inform decisions to expand navigation, analytics, or care coordination tools to other boroughs and organ types, creating future demand for specialized care management platforms, analytics consulting, and workforce training tailored to transplant navigation.

The pilot is part of a Doris Duke grant-funded evaluation of eGFR algorithm changes and transplant access; expansion decisions will likely follow analysis of pilot results.
