Initial term through July 31, 2027; automatically renews year-to-year unless either party provides written notice of non-renewal at least 60 days prior to the then applicable termination date. Either party may terminate for convenience with 90 days’ written notice, or for default after a 30-day cure period. Provider may terminate for insufficient enrollment as described in Section 4.
