Initial three‑year term starting July 7, 2026 with automatic one‑year renewals if no 90‑day non‑renewal notice is given; agreement continues in additional 12‑month periods after completion of initial and renewal terms. Provider may renegotiate or terminate upon 30 days’ notice if regulatory or operational changes adversely affect profitability.
