Jackson County approved an MOU with Cal Poly Corporation, dba Cal Poly Partners, to participate in the Pathways to Health Project using a 'Food is Medicine' model to help prevent gestational diabetes from progressing to type 2 diabetes. The agreement enables the County to refer eligible families into the program.
