The Board of Health is authorizing amendments to the Healthy Dane Collaborative MOU that govern the Community Health Data Platform contract with Conduent Healthy Communities Corporation, while planning not to renew that contract and to transition to a new vendor. Conduent currently provides the hosted data system at healthydane.org used for community health assessment and planning.
