Effective on the date of last signature; expires August 31, 2027, with DSHS option to renew for up to one additional year and to extend up to 12 additional months in one-month intervals

Initial term runs from the effective date (date of last signature) through August 31, 2027. DSHS may, at its sole discretion, renew the contract for up to one additional year for a maximum contract term of five years and may, with at least 30 days’ written notice, extend the contract as necessary for up to 12 additional months in one-month intervals at the then-current contract rate(s).
