State Mandates: $87/hr per nurse and $375 coordination fee; $96/hr for Health Assessment and evaluation (baseline $240 per case, not to exceed 150 minutes per student); $240.50 per IEP “one stop fee”; $85/hr for daily office coverage; $87/hr for RN; $82/hr for LVN; $77/hr for CNA; $375 for Growth & Development, Epipen Training & Narcan plus $50 per each additional school (same date); $350 for other in‑services; Health Cards $0.40 each plus shipping & handling; 20% late penalty every 30 days after 60 days past due

Effective upon signature by both parties and continuing until terminated by either party with 30 days written notice

Contract continues on an ongoing basis until either party provides 30 days written notice of termination; rates or policies may be modified only by written notification signed by both parties or by electronic acknowledgement from the client agreeing to changes.
